Article R - Change of Name, Address, Marital Status, Beneficiary

It is the responsibility of each employee to keep his direct supervisor informed when changes in name, address, or telephone numbers occur. It is also the responsibility of the employee to notify the library’s business manager of any change of address, name, marital status, telephone number, etc. as soon as the change becomes effective.
